Out of Country Emergency Medical Insurance is suggested for all OSA teams traveling outside of Ontario. The coverage is available to OSA teams at a preferred rate of $2.15 per person (up to age 60) per day. Please allow 48 hours for processing.
Coverage Details
Application & Purchase Process
- Complete a Travel Insurance Manifest noting travel dates, and names and birthdates of all travelers. This insurance is not limited to OSA members; parents, siblings, etc. may be included in the list of travelers
- Email the completed Travel Manifest to Belinda.Antonio@hkmb.com.
- The Travel Manifest will calculate the total premium automatically. The premium can be paid by credit card or cheque. If paying by credit card, please provide a telephone number where you can be reached. HKMB will call you to obtain details and coordinate payment directly with Travel Underwriters on your behalf.
- Confirmation of coverage and the policy number will be provided to you via e-mail within 48 hours.
- Once confirmation is received, print the Coverage Summary and add the following information to the Wallet Card at the bottom of the Summary:
- Policy Number
- Name of team
- Departure and return dates
Cheques should be made payable to Travel Underwriters. Please reference the policy number on the cheque and mail to:
